The parents of an eight-month-old baby, Willow Katarina Horne, are seeking answers after her death at Kalgoorlie Health Campus last year. Despite a clinical review of her care, no definitive cause of death has been established. According to ABC News, Willow died on September 9, 2025, shortly after being admitted with respiratory issues.

Willow’s mother, Kaydence Sutherland, expressed her belief that the family deserves clarity on the circumstances surrounding her daughter’s death. She noted that she feels failed by the healthcare system, suggesting that had Willow been transferred to Perth earlier, her chances of survival might have been greater.

A panel led by Dr. Helen Van Gessel, Executive Director of Clinical Excellence at WA Country Health Service, conducted the review and found that Willow was continuously monitored during her hospital stay. The review indicated that while a confirmed cause of death was not identified, myocarditis, an inflammation of the heart muscle, was suspected. Dr. Van Gessel emphasized that this condition is rare and challenging to diagnose, particularly in young children.

The review also highlighted limitations in the aeromedical retrieval network that hindered Willow’s transfer to Perth Children’s Hospital. Three care management issues were identified, including insufficient consideration of alternate diagnoses and inconsistent responses to her deterioration. The panel’s report includes seven recommendations aimed at improving future care.

WA’s Deputy Premier Rita Saffioti expressed sympathy for Willow’s family and indicated that a coronial inquest into the death would be supported. The family hopes this inquest will provide the answers they seek regarding the tragic loss of their daughter.
