Errors in Mental Health Care Linked to Young Man’s Suicide

A review by Albury Wodonga Health has uncovered significant errors in the mental health care provided to Blake Whittington, a 23-year-old man who died by suicide in December 2026, just one day after being discharged from Albury Hospital. According to ABC News, the investigation revealed a series of administrative and referral mistakes that contributed to the tragic outcome.

Whittington was initially taken to the hospital following a suicide attempt and was assessed as requiring an urgent mental health response. However, after being downgraded to a less urgent classification, he was discharged without adequate follow-up care. Alarmingly, the mental health worker responsible for his case was on a day off at the time of his discharge, leading to a critical gap in support.

His mother, Tanya Caputo, expressed deep concern over the lack of proper care and the system’s failure to respond effectively to her son’s needs. She stated that he should have been admitted to the hospital for further treatment rather than being sent home so soon after his suicide attempt. Caputo highlighted that missed opportunities in the mental health system ultimately cost her son his life.

The review identified a lack of governance and proper procedures for internal referrals as key issues that led to the downgrading of Whittington’s case. It also noted that the clinician responsible for his assessment had not received adequate training and made several administrative errors, including incorrect details on the assessment form. These mistakes contributed to the inappropriate referral to a less urgent service, which failed to contact him until after his death.

In light of these findings, recommendations have been made for improved training and screening of mental health staff at Albury Wodonga Health. The organization has stated that it is cooperating fully with the ongoing coronial investigation into Whittington’s death and has extended condolences to his family.
